本人（或本人作为未成年人之法定监护人）知悉并确认，参与由南北卡匹克网球协会组织的网球/匹克球比赛、训练、培训课程、夏令营、集训、体验课及任何相关活动，存在一定的风险，包括但不限于身体受伤、意外事故、疾病、财产损失等。本人理解并自愿承担因参与上述活动所产生的全部风险与责任。

在法律允许的最大范围内，本人同意免除并放弃对主办方、承办方、组织者、教练、工作人员、志愿者、场地方及其关联人员因本人或本人子女参与活动而产生的任何直接或间接损失、伤害或责任的追究权利（因故意或重大过失造成的损害除外）。

本人确认（或确认本人子女）身体健康状况适合参加上述体育活动，如存在任何健康问题或特殊情况，已事先如实告知主办方。因隐瞒健康状况、未遵守活动规则或不当行为所造成的一切后果，责任由本人自行承担。

​

本人同意主办方在活动期间为教学、宣传或记录目的，合理使用本人或本人子女的照片、视频或影像资料（如不同意，可提前书面告知主办方）。

​

本人确认已认真阅读并完全理解本免责声明的内容，自愿加入协会成为会员并同意其法律效力。本免责声明自本人注册协会会员之日起生效。

I (or I, as the legal parent or guardian of a minor) acknowledge and confirm that participation in tennis and/or pickleball tournaments, practices, training programs, camps, clinics, trial sessions, and any related activities organized by Carolina Asian Pickleball & Tennis Association involves inherent risks, including but not limited to personal injury, accidents, illness, and property damage. I fully understand these risks and voluntarily assume all risks and responsibilities arising from participation in such activities.

 

To the fullest extent permitted by law, I hereby agree to release, waive, and discharge the organizers, sponsors, hosts, association, coaches, instructors, staff members, volunteers, facility owners, and their affiliated parties from any and all claims, liabilities, losses, damages, or injuries, whether direct or indirect, arising out of or related to my or my child’s participation in the above activities, except where such damages are caused by willful misconduct or gross negligence.

I confirm that I (or my child) am in good physical health and medically fit to participate in the above activities. Any known medical conditions or special circumstances have been truthfully disclosed to the organizers in advance. I agree that any consequences resulting from failure to disclose health conditions, failure to follow rules, or improper conduct shall be my sole responsibility.

I grant permission to the organizers to use photographs, videos, or other media images of me or my child taken during activities for instructional, promotional, or record-keeping purposes. If I do not consent to such use, I understand that I must notify the organizers in writing in advance.

I confirm that I have carefully read and fully understand this Liability Waiver and Disclaimer. I voluntarily agree to join the Association as a member and acknowledge that this waiver is legally binding. This disclaimer shall become effective on the date of my membership registration with the Association.
